Geriatric Care Management*
*The Geriatric Care Manager specializes in the care and needs of the elderly. They help families adjust and cope with the challenges of aging by; conducting care planning assessment, coordinating and monitoring in home services, acting as a liaison to families at a distance, making sure things are going well and alerting families to problems, assisting with moving an older person to or from a retirement complex, care home, or nursing home and providing consumer education and support. We partner with the client and their loved ones, focusing on individuality and utilizing a preventive approach which will allow the client to stay independent and in control of their life.

Peaceful Living believes in utilizing a team of care professionals to provide a holistic and rewarding approach to individuals and their families. The services we provide work in relation to each individual’s environment, needs, health, independence, family and community support. Our staff members are at the heart of our service, offering compassion and respect in assisting the individual to maintain their dignity and autonomy in our Adult Family Homes.
We provide 24 hour staffing which includes personal and direct care, personalized to each individual. Our staff is trained to meet the needs of those with developmental and physical disabilities, including medical and behavioral challenges, as well as the aging population. An individual must be eighteen years of age to reside in an Adult Family Home.
Come take a tour and see either a one or two level Town House each with a two- car attached garage, with two or three bedrooms and handicap accessible roll in shower on the lower level. Second level includes one bedroom, loft and full bath. Gas fireplace in living room with vaulted ceilings (two levels) give a warm and “homey” feeling throughout. Kitchen includes microwave, dishwasher, washer/dryer all on lower level with central air and patio. Town Houses are located in New Richmond and Hudson.
